Who we see
We see patients with diabetes and at least two complications, such as kidney disease and autonomic neuropathy.
Appointments may be face-to-face, by phone, or by video. This is decided based on what is most beneficial to you and your care.

We see patients with any type of diabetes and with two or more of the following:
- renal disease – CKD stage 3 onwards, excluding end stage renal disease and transplant patients
- autonomic neuropathy and gastroparesis
- cirrhosis or previous liver transplant
- heart failure NYHA more than 2
Other related services
This service is only for patients who have a confirmed diagnosis of diabetes with complications.
- For patients with acute complications of diabetes (DKA or HHS) and severe hypoglycaemia, refer urgently to the admitting medical team at King’s College Hospital.
- Our community diabetes team provides optimisation for people with type 2 diabetes and HbA1c more than 58mmol/mol despite two or more therapies. They also provide specialist input for diabetic kidney disease and for cardiovascular disease prevention. This includes people with eGFR 30-60ml/min or ACR more than 30mg/mmol despite treatment or known heart failure.
Suggested investigations
HbA1c test and full blood profile (renal and liver, full lipid profile) and urine ACR/PCR within three months of the referral being made.
