We help patients in understanding and managing their heart condition, whilst providing support with making appropriate lifestyle changes to improve both heart health and overall quality of life.
We provide a comprehensive cardiac rehabilitation service for local patients and their families and carers, offering individual advice, support and information. Our menu of services are delivered via face-to-face or remote appointments based at King’s College Hospital, Tessa Jowell Health Centre and local community leisure centres.
Following heart attack, angioplasty, coronary artery by-pass grafts, heart valve surgery and heart failure, patients are offered a tailored cardiac health programme of exercise, education and psychological support.
